The incumbent is responsible for ensuring that enhanced care and services, including intensified treatment, safety planning, and follow-up care for missed appointments, are provided for high-risk Veterans; and providing consultation to care providers actively involved with treating Veterans at elevated risk for suicide.

Collaborates with treating clinicians to assess risk and provide guidance about criteria for Patient Record Flags Category I-High Risk for Suicide (HRS-PRF) placements and ensures that high risk Veterans are receiving appropriate care following their HRS-PRF placements, including increased frequency of clinical care visits and follow-up after missed appointments.

This may include making personal contact with identified high-risk Veterans (e.g., letters, face-to-face, telephone).

The incumbent ensures Veterans have engaged in safety planning and that safety plans are updated as appropriate by using the tracking system created by the SPP.

Offers consultations to treating clinicians to assist them with managing Veterans at high risk for suicide.

The incumbent performs comprehensive suicide risk assessments and develops and integrates safety planning and safety goals into the Veteran's collaborative, individualized treatment plan.

They provide Advanced Directive information and education including but not limited to the following:

Durable Power of Attorney for Health Care, Living Will, Health Care Directive, and/or treatment preferences document to Veteran and families.

Also assists the Veteran in preparing such documents, as requested.

Assists the interdisciplinary team with identifying appropriate decision makers if Veteran is unable to make own decision (Next of Kin, HCPOA, Legal Guardian).

Facilitates advanced care planning group visits with Veterans and support system (as appropriate). See VHA Handbook , Advance Care Planning and Management of Advance Directives.

In conjunction with the healthcare team, the incumbent identifies and assesses risk, safety, supports and implements appropriate interventions/treatment for those who are experiencing abuse, neglect, exploitation or are experiencing domestic or intimate partner violence.

Adheres to VA and mandated reporter requirements. Advises and collaborates with interdisciplinary teams throughout the medical center on abuse, neglect, exploitation, and intimate partner violence issues.

Access and Referrals:

The incumbent is responsible for responding to referrals from staff members, the Veterans Crisis Line (VCL), and other touch points to ensure that at-risk Veterans immediately receive care and services.

This is completed by s/he being a highly accessible point of contact and subject matter expert for the SPP.

The incumbent will respond to consults from the VCL within 1 business day of receipt and ensure consults are closed within 3 business days.

Consult responses may include assisting Veterans with the enrollment process if they are new to VA, facilitating the resolution of the Veteran's needs identified in the consult and reassessing for any potential risk, and making consults to care team members as appropriate.

Monday to Friday, 7:00 am - 3:30 pm

Pay:

Competitive salary and regular salary increases When setting pay, a higher step rate of the appropriate grade may be determined after consideration of existing pay, higher or unique qualifications, or special needs of the VA (Above Minimum Rate of the Grade).

Paid Time Off: 37-50 days of annual paid time offer per year days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)
Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.

Parental Leave:

After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.


Child Care Subsidy:
After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $
