ROLE & RESPONSIBILITIES:
The primary aim of this position is to avert child maltreatment and unnecessary out-of-home placements while fostering positive family dynamics for youth aged 0-18 and their families.
KEY DUTIES INCLUDE:
- Develop and sustain professional relationships with clients, service providers, funding agencies, and colleagues.
- Collaborate within a team to deliver case management, crisis intervention, and coordinate referrals to additional services.
- Ensure family engagement in community programs by facilitating connections and maintaining ongoing communication between families and program staff.
- Engage non-custodial parents by providing support services aimed at re-establishing family connections.
- Maintain consistent communication with all family service providers to guarantee seamless care.
- Conduct regular home visits to assess needs and provide support.
- Organize and attend treatment reviews and meetings with service providers.
- Facilitate psycho-educational groups as opportunities arise.
- Collaboratively develop service plans and treatment objectives with clients.
- Supervise visits between children and caregivers as necessary, providing reports to relevant authorities.
- Participate in professional development and contribute to program enhancements.
- Ensure timely completion and submission of all necessary documentation.
- Adapt work hours to meet family needs and attend relevant community events.
- Build relationships with educational institutions to monitor and address school attendance and related issues.
- Maintain regular communication with the C.A.R.E. program to support children's success.
- Perform additional duties as assigned by the Supervisor of Service Navigation.

QUALIFICATIONS:
- Associate's or Bachelor's degree preferred, with a minimum of one year of direct experience working with children and families.
- Experience engaging with individuals and families from diverse cultural and socioeconomic backgrounds.
- Ability to demonstrate understanding and appreciation of clinical concepts that focus on family strengths.
A valid NYS Driver's License is required.
