+ AdventHealth Apopka is a modern, seven-story, 120-bed facility offering a wide array of services, including advanced surgical options and a catheterization lab.
+ The hospital features a 30,000 square-foot Emergency Department equipped with pediatric-friendly beds, imaging services, a cafeteria, gift shop, and an on-site fire station.
+ This facility opened to the community, replacing a long-standing 50-bed hospital that served the area for over forty years.
Your Role:
*The Care Management Supervisor operates under the general guidance of the Care Management Facility Director, overseeing daily departmental operations and supervising team members.

Key Responsibilities:

Coordination of Care, adherence to Centers for Medicare & Medicaid Services (CMS) Conditions of Participation, and implementation of Social Services processes based on data management. Responsibilities include accountability for Length of Stay, Excess Days, Avoidable Days, Readmission Prevention, and Discharge Planning.

Additionally, the Care Management Supervisor is responsible for departmental engagement, enhancing customer experience, ensuring compliance with regulatory standards, and conducting employee evaluations.

This role fosters professional and collaborative relationships with the Interdisciplinary Team to support process improvements and achieve hospital metrics.

Value You Bring to the Team:

Oversee daily operations of the Care Management department (both Inpatient and Emergency Department settings). Implement Corporate Care Management strategies focused on customer experience. Ensure documentation compliance within the Care Management department (Discharge Planning Evaluation, IMM, MOON, Patient Choice, etc.). Identify and utilize resources to facilitate information sharing among staff. Actively participate in daily Multidisciplinary Rounds, ensuring staff engagement in discussions regarding patient transitions to post-acute settings and addressing barriers to care plans. Ensure staff documentation adheres to guidelines, providing coaching as necessary. Regularly assess patients for post-hospital needs and ensure documentation occurs within one calendar day of admission. Advocate for timely care and escalate any delays. Collaborate with patients, family caregivers, care teams, and payors to develop effective discharge plans that meet patient needs, comply with regulatory requirements, and support organizational objectives related to readmission reduction. Support the implementation of processes for identifying at-risk populations and acquire necessary resources/services to enhance patients' self-management success.
Quality Improvement:
Seek opportunities to enhance the quality of care/services. Report relevant quality/risk issues to appropriate individuals, departments, and committees promptly. Participate in performance improvement teams as needed.

Monitor and document ongoing activities to validate the effectiveness of care management efforts, ensuring compliance with external agencies. Assist in developing and overseeing the implementation of Care Management policies, procedures, and standards.

Plan, organize, and delegate activities to ensure safe, efficient, and effective unit operations, collaborating with other hospital and leadership staff to achieve these goals.

Conduct regular staff meetings, encouraging open communication and participation in decision-making. Maintain strong professional relationships with facility and divisional leadership.

Communicate and collaborate across the Care Management team regarding data management, performance improvement, care coordination, care transition management, medication management, and social services. Monitor and meet productivity standards and staffing needs.

Foster a work environment conducive to staff retention. Manage personnel functions, including staffing, performance reviews, and corrective actions. Promote professional growth and development among employees. Maintain a predictable schedule, including availability on weekends and holidays. Cultivate collegial relationships with staff, physicians, and other departments. Maintain open lines of communication with staff.

Recruit and retain individuals with the necessary skills, working to develop staff to their fullest potential.
Qualifications for Success:

Minimum Requirements:


Registered Nurse:

BSN or

Social Worker:
BSW or Master's in a related field with 3 years of direct Care Management/Utilization Management experience in an acute care environment.
Preferred Qualifications:+

BSN with a Master's in a related field+

Social Worker:
