Position Overview:

The Care Management Manager operates under the general supervision of the Care Management Facility Director, overseeing daily departmental operations and managing team members. Key responsibilities include:

  • Coordination of Care and adherence to CMS Conditions of Participation
  • Implementation of Social Services processes based on data management
  • Accountability for Length of Stay, Excess Days, Avoidable Days, Readmission Prevention, and Discharge Planning
  • Enhancing department engagement and customer experience
  • Ensuring compliance with regulatory standards and conducting employee evaluations

Key Contributions:

The Care Management Manager plays a crucial role in supporting the daily functions of the Care Management department across both inpatient and emergency settings. Responsibilities include:

  • Implementing Corporate Care Management strategies with a focus on enhancing customer experience
  • Ensuring compliance with documentation standards in the Care Management department
  • Facilitating daily Multidisciplinary Rounds to discuss patient transitions and address barriers
  • Advocating for timely care and effective discharge planning in collaboration with patients, families, and care teams
  • Identifying at-risk populations and acquiring necessary resources for successful self-management

Quality Improvement:

Actively seeking opportunities to enhance the quality of care and services, the Care Management Manager will:

  • Report quality and risk issues promptly to relevant stakeholders
  • Participate in performance improvement initiatives
  • Monitor and document care management activities to ensure compliance with external agencies

Leadership Responsibilities:

In this role, you will assist in developing and implementing Care Management policies and procedures, while:

  • Planning and organizing departmental activities for efficient operations
  • Holding regular staff meetings to encourage information sharing and collaborative decision-making
  • Maintaining strong professional relationships with leadership and staff
  • Overseeing personnel functions, including staffing and performance evaluations

Qualifications:

Minimum Requirements:

  • Registered Nurse: BSN or
  • Social Worker: BSW with a Master's in a related field
  • 3 years of direct Care Management/Utilization Management experience in an acute care environment

Preferred Qualifications:

  • Registered Nurse: BSN with a Master's in a related field
  • Social Worker: MSW and LCSW
  • 2 years of supervisory or managerial experience
  • RN-BC Certification in Care Management
  • LCSW
