Job Summary:

The Patient Educator plays a vital role in improving health outcomes for patients with chronic diseases. As a key member of the healthcare team, this individual will work closely with patients, healthcare providers, and other stakeholders to coordinate care, educate patients, and enhance communication and continuity of care.

Key Responsibilities:
  • Advocacy and Coordination: Acts as an advocate for patients' care needs, identifying opportunities for care interventions and addressing deficits and gaps in care.
  • Care Coordination: Coordinates care services for patients with chronic diseases, such as diabetes, hypertension, and asthma, in accordance with established clinic policies and procedures.
  • Patient Education: Implements targeted strategies to improve health, functional, or quality of life outcomes, including disease management and patient teaching plans.
  • Progress Monitoring: Routinely monitors patients' status, needs, and progress, and identifies reasons for static or regressive progress to proactively encourage adjustments to their plan of care.
  • Team Collaboration: Participates in patient care coordination, pre-visit planning, huddles, one-on-one communication, and documentation of services accessed and health care-related concerns/needs.
  • Goal Setting: Discusses self-management goals with patients, assists them in selecting appropriate goals, and creates an action plan to achieve them.
  • Documentation and Follow-up: Monitors and documents goals selected and follows up with patients to determine if goals are achievable and provides assistance when needed.
Requirements:
  • Education: High school diploma or equivalent required; completion of an accredited Licensed Practical Nursing program or RN program; Active Florida LPN or RN license.
  • Experience: Minimum of two (2) years' experience as a Licensed LPN or RN in Healthcare or Managed Care is preferred.
  • Skills: Knowledge of health implications of chronic diseases and risks, excellent oral and written communication skills, high level of initiative and leadership, good advocacy skills, and ability to handle multiple projects and set priorities.
