Role Overview:
The Care Management Supervisor operates under the general guidance of the Care Management Facility Director, overseeing daily departmental operations and supervising team members.

Key Responsibilities:

The Care Management Supervisor is accountable for, but not limited to:
Coordination of Care, adherence to Centers for Medicare & Medicaid Services (CMS) Conditions of Participation, and implementation of Social Services processes based on data management. Responsibilities include managing Length of Stay, Excess Days, Avoidable Days, Readmission Prevention, and Discharge Planning.

Additionally, the Supervisor is responsible for fostering department engagement, enhancing customer experience, ensuring compliance with regulatory standards, and conducting employee evaluations.

This role also involves supporting collaborative relationships with the Interdisciplinary Team to drive process improvements and achieve hospital performance metrics.

Value Proposition:

Facilitates the daily operations of the Care Management department across both Inpatient and Emergency Department settings. Implements Corporate Care Management strategies with a focus on enhancing customer experience. Ensures compliance with documentation standards within the Care Management department (including Discharge Planning Evaluation, IMM, MOON, Patient Choice, etc.). Identifies and utilizes resources to enhance information sharing among staff. Actively participates in daily Multidisciplinary Rounds, ensuring staff engagement on their assigned units to discuss patient transitions to post-acute settings and address barriers to care plans. Monitors staff documentation for adherence to guidelines and provides coaching as necessary. Assures that patients are regularly assessed for post-hospital needs and that documentation occurs promptly upon admission. Advocates for timely care progression and ensures that discharge plans are collaboratively developed with patients, family caregivers, care teams, and payors. Supports effective discharge planning to meet patient needs, comply with regulatory requirements, and achieve organizational objectives related to readmission reduction. Aims for continuous improvement in quality of care and services, reporting relevant quality/risk issues to appropriate stakeholders in a timely manner. Participates in performance improvement initiatives as needed.

Monitors and documents ongoing activities to validate the effectiveness of care management efforts in achieving compliance with external agencies. Assists in developing and overseeing the implementation of Care Management policies, procedures, and standards. Plans, organizes, and delegates activities necessary for safe, efficient, and effective unit operations, collaborating with other hospital and leadership staff to achieve these goals. Conducts regular staff meetings, encouraging open communication and staff involvement in decision-making. Maintains strong professional relationships with facility and divisional leadership.

Communicates and collaborates across the Care Management team in areas such as data management, performance improvement, care coordination, care transition management, medication management, and social services. Monitors productivity standards and staffing requirements, fostering a work environment conducive to staff retention. Responsible for personnel functions, including staffing, performance reviews, and corrective actions. Promotes professional development and growth among employees. Maintains a consistent schedule, including shifts on weekends and holidays, while nurturing collegial relationships with staff, physicians, and other departments.

Recruits and retains individuals with the necessary skills and focuses on developing staff to reach their full potential.

Qualifications for Success:

Minimum Requirements:

Registered Nurse: BSN or

Social Worker: BSW or Master's in a related field with 3 years of direct Care Management/Utilization Management experience in an acute care environment.
Preferred Qualifications:

Registered Nurse: BSN with a Master's in a related field.
Social Worker: MSW, LCSW with 2 years of supervisory or managerial experience, RN-BC Certification in Care Management, LCSW.
