About the Role: Phamily is seeking a dedicated Patient Care Coordinator to join our team. This role is pivotal in enhancing patient engagement and ensuring seamless care delivery through our innovative platform.

Key Responsibilities:

  • Assist in the development of patient-centered care strategies that align with primary care practices.
  • Collaborate effectively with healthcare teams to streamline processes that enhance patient care.
  • Utilize the Phamily platform to support patients with chronic conditions and facilitate their care journey.
  • Engage in quality improvement initiatives aimed at optimizing population health outcomes.
  • Maintain patient registries and provide essential data to healthcare teams to meet patient needs.
  • Support practice staff in creating proactive management interventions for targeted patient populations.
  • Ensure a positive patient experience through professional communication and efficient routing of inquiries.
  • Identify and report any discrepancies in patient data to the appropriate personnel.
  • Participate in team meetings and contribute to a collaborative work environment.
  • Adhere to all relevant policies and procedures in performing job functions.
  • Monitor patient attribution to ensure accurate alignment with care teams.
  • Perform additional duties as assigned.

Required Qualifications:

  • A minimum of three years of experience in a relevant healthcare specialty, with a preference for population health experience.
  • Strong problem-solving skills and the ability to manage multiple tasks effectively.
  • Exceptional communication skills, both verbal and written.
  • Certification as a Medical Assistant from a recognized organization or LPN; significant experience in a primary care setting may be considered in lieu of certification.

Preferred Qualifications:

  • Experience with EHR/EMR systems is highly desirable.
  • Bi-lingual candidates, particularly in English and Spanish, are preferred.

Compensation and Benefits:

  • Hourly wage ranging from $20 to $25, based on experience.
