About the Role:

The Patient Care Manager Assistant (LPN) is a key member of our team at Caretenders, a part of LHC Group. We are committed to providing high-quality care to our patients, and this role plays a crucial part in ensuring that we deliver on that promise.

Responsibilities:

  • Assist the Patient Care Manager in coordinating a team of staff to provide high-quality care to home health patients.
  • Ensure that the team is committed to providing quality care in an efficient manner, in accordance with physician's orders and State/Medicare guidelines.
  • Prepare patient lists and necessary documents for weekly case conferences.
  • Provide direct patient care, as necessary, in accordance with scope of practice, plan of care, and physician orders, in coordination with other members of the healthcare team.
  • Assist the Patient Care Manager in clinician documentation review/audits to ensure complete, accurate, and timely submission of all documentation by field staff.

Requirements:

  • Current CPR certification.
  • Current LPN licensure in the state of practice.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
