Job Summary

CN Guidance and Counseling Services is seeking a highly motivated and compassionate Peer Advocate Specialist to join our team. As a key member of our recovery services team, you will provide support, advocacy, and linkage services to individuals and families affected by substance use disorders.

Key Responsibilities
  • Engage with individuals in emergency departments and provide brief person-centered service plans, referrals, and linkages to needed services.
  • Offer outreach, advocacy, mentoring, and recovery support services to those seeking or sustaining recovery.
  • Collaborate with hospital staff to assess individual needs and connect them to treatment and support services.
  • Provide follow-up support and services to individuals and families at 30, 60, 90, 120, and 180 days post-discharge.
  • Collect data to assist in reporting to funding sources and hospital systems.
  • Act as a liaison between the program and the communities we serve.
  • Represent the program in a professional and knowledgeable manner.
  • Assist in promoting the program through information and education about its services.
  • Provide direct outreach to courts, attorneys, hospitals, probation, parole, DSS, CPS, DDP, MDs, schools, and other community groups.
  • Represent the program at community events, such as Narcan trainings, fairs, forums, etc.
  • Document and report on contacts made in the community.
  • Complete quality improvement training and participate in refresher training as needed.
  • Facilitate sharing of data with individuals to facilitate partnered decision-making.
  • Encourage quality improvement processes and provide consistent resources for QI and professional development.
  • Adhere to all safety protocols and procedures to ensure a safe working environment.
Requirements
  • High school or equivalent education.
  • Relevant life experience, including personal experience living with a disability, history of substance use, or social/emotional challenge.
  • Valid driver's license.
