POSITION SUMMARY:

The Community Outreach Specialist plays a crucial role within an individual's treatment team, focusing on outreach, support, and care coordination for eligible clients. This position serves as a health coach and role model, while also promoting the Agency's services within the community. The Specialist collaborates with agency staff and external organizations to raise awareness about drug and alcohol prevention services.


KEY RESPONSIBILITIES:

  • Conduct outreach efforts to engage clients in services and provide additional support for their participation in Medication-Assisted Treatment and community-based mobile services.
  • Offer health coaching and recovery support, addressing self-advocacy, wellness management, medication adherence, smoking cessation, entitlements, and daily living skills.
  • Facilitate transportation for clients to medical and other appointments to enhance their engagement and follow-through.
  • Deliver concrete services to clients, including benefits enrollment as required by program needs.
  • Maintain organized and timely documentation, records, statistics, and reports in accordance with agency policies.
  • Coordinate care planning with other service providers to ensure collaborative and goal-oriented care, including transitions of care.
  • Act as a resource for team members regarding substance abuse issues.
  • Provide outreach and service planning through both telephonic and in-person interactions in the community.
  • Link clients to community services, including medical, behavioral, residential, and entitlement resources as outlined in their care plans.
  • Monitor service delivery to ensure coordination and continuity, advocating with service providers as necessary.
  • Provide crisis intervention and follow-up support.
  • Complete Quality Improvement (QI) training during onboarding and participate in ongoing QI training as required.
  • Serve as a liaison between the Agency and the communities served.
  • Represent the Agency professionally and knowledgeably.
  • Assist in promoting the Agency's services through community education and outreach.
  • Engage directly with courts, attorneys, hospitals, probation, parole, and various community groups for referrals to the Agency.
  • Report to and collaborate with the director(s) on outreach activities.
  • Represent the Agency at community events, including trainings and forums.
  • Document community contacts and report findings to the director(s), including contact details.
  • Drive the Company Vehicle as required.
  • Identify and implement quality improvement initiatives as part of professional development.
  • Foster a culture of inclusion that values diverse strengths and experiences among employees.
  • Adhere to safety protocols to maintain a safe working environment for all staff.
  • Perform other duties as assigned.

QUALIFICATIONS:

  • High School Diploma or GED.
  • Certified Recovery Peer Advocate (CRPA) Certification required (or provisional certification upon hire with full certification within one year).
  • Understanding of medication-assisted treatment, opiate overdose prevention, and substance use disorder recovery services is essential.
  • Relevant personal experience, such as living with a disability or history of substance use, is required to effectively support clients' recovery goals.
  • Familiarity with community resources and outreach strategies is preferred.
  • Valid New York State Driver's License with a clean DMV record.
