Job Overview

POSITION SUMMARY:

The Certified Peer Specialist plays a crucial role within a CCBHC Support Team, dedicated to providing outreach, assistance, and care coordination for eligible residents. This position serves as a role model and health coach, guiding clients on their journey to recovery.

QUALIFICATIONS:

  • High School Diploma or GED required.
  • Certification (CERPA-P) must be obtained within one year of employment.
  • Relevant personal experience is essential, particularly in living with a disability or facing social/emotional challenges, with a commitment to using this experience to support others in their recovery journey.

ESSENTIAL DUTIES:

  • Engage with clients to encourage their participation in services and provide necessary support for successful community living.
  • Offer health coaching and recovery assistance, focusing on self-advocacy, wellness management, medication adherence, smoking cessation, entitlements, and daily living skills.
  • Accompany clients to medical and other appointments to ensure their engagement and follow-through.
  • Deliver concrete services to clients, including benefits and enrollment assistance as required by the program.
  • Maintain accurate and organized documentation, records, and reports in compliance with policies and procedures.
  • Collaborate with other service providers to coordinate care planning, ensuring a goal-oriented and cooperative approach.
  • Serve as a resource for team members regarding psychosocial and substance abuse issues.
  • Conduct outreach and service planning both telephonically and in-person within the community.
  • Facilitate connections to community resources, including medical, behavioral, residential, and entitlement services as outlined in the interdisciplinary care plan.
  • Monitor service delivery to clients to guarantee continuity and advocate with service providers as necessary.
  • Provide crisis intervention and follow-up support.
  • Foster an inclusive culture that values the diverse strengths, perspectives, and experiences of all employees, enhancing decision-making and innovation.
  • Collaborate with the CN Guidance team to implement the Plan-Do-Study-ACT continuous quality improvement framework to tackle systemic challenges.
  • Complete Quality Improvement training during onboarding and participate in refresher courses as needed.
  • Identify and engage in quality improvement initiatives as part of professional development.
  • Encourage quality improvement processes and provide ongoing resources for professional growth.
  • Commit to maintaining a safe work environment by adhering to established safety protocols and procedures.
  • Perform additional duties as assigned.
