Job Overview

POSITION SUMMARY

The Child and Family Care Coordinator is an integral member of a multidisciplinary team dedicated to providing comprehensive care management for children and their families facing mental health challenges and/or concurrent substance use issues. This role aims to enhance therapeutic outcomes and foster peer support to promote sustained recovery. The Coordinator will prioritize the essential needs of trauma-affected children and families, ensuring that these needs are adequately addressed.

EDUCATIONAL BACKGROUND:

A Bachelor's or Master's Degree in relevant fields such as Social Work, Psychology, Education, Rehabilitation, Occupational Therapy, Counseling, Community Mental Health, Sociology, Speech and Hearing, or Physical/Recreational Therapy is preferred. Degrees in other related disciplines may also be considered.


PROFESSIONAL EXPERIENCE:

  • For candidates with a Bachelor's degree, a minimum of one (1) year of relevant human services and direct service experience is required.
  • Experience should include providing direct support to individuals with severe mental health conditions, co-occurring substance use disorders, developmental disabilities, and/or physical impairments, as well as linking clients to a broad spectrum of essential services for independent community living and advocating for marginalized populations.
  • Bilingual candidates are preferred.
  • Proficiency in computer software and electronic health record systems is necessary.
  • Strong written, verbal, and computational skills are essential for documenting and presenting records in line with program standards.
  • Experience and a thorough understanding of the interdisciplinary planning process and developmental treatment models are required.
  • Knowledge of Medicaid, Social Security, and other entitlement programs is preferred.
  • Exceptional interpersonal skills are mandatory.
  • Understanding of cultural competence and sensitivity is crucial.
  • Willingness to travel regularly, including transporting clients in agency vehicles, is required. A valid driver's license is necessary for certain regions.

KEY RESPONSIBILITIES:

  • Conduct initial and ongoing assessments of assigned clients to document their strengths, needs, goals, and available resources.
  • Collaborate in the development, documentation, review, and updating of client-centered, comprehensive, interdisciplinary care plans in consultation with the Program Supervisor and other team members to ensure focus on desired outcomes.
  • Maintain effective communication with clients, primary care providers, mental health professionals, family members, and other relevant parties on behalf of clients.
  • Organize and maintain documents, records, statistics, and related reports in a timely and accurate manner as per established policies.
  • Coordinate care planning with other service providers to ensure goal-directed, collaborative care, including transitions of care.
  • Participate actively in team meetings to provide insights on psychosocial conditions and review client progress, plans, and outcomes.
  • Serve as a resource for team members regarding psychosocial and substance use issues.
  • Provide outreach, engagement, and service planning through both telephonic and face-to-face interactions.
  • Facilitate connections to community services, including medical, behavioral, residential, and entitlement services as outlined in the interdisciplinary care plan.
  • Monitor service delivery to clients to ensure continuity and advocate with service providers as necessary.
  • Provide crisis intervention and follow-up support as needed, available 24/7.
  • Engage in ongoing supervision, training, and education to maintain high-quality service delivery and professional development.
  • Encourage data sharing with clients to promote informed decision-making and keep them updated on their progress.
  • Participate in quality improvement training and initiatives as part of professional development goals.
  • Foster a culture of inclusion and respect for all employees, valuing their individual strengths and experiences.
  • Adhere to safety protocols to ensure a safe working environment for all employees.
  • Other duties as assigned.
