POSITION SUMMARY:

  • The Community Engagement Coordinator plays a vital role within an individual's care team, focusing on outreach, support, and the seamless coordination of services for eligible clients. This position serves as a mentor and health coach, guiding clients through their recovery journey. Additionally, the Coordinator is tasked with promoting the organization and its initiatives within the community, collaborating with internal staff and external agencies to raise awareness about drug and alcohol prevention services.

KEY RESPONSIBILITIES:

  • Conduct outreach to clients to encourage their involvement in services and provide supplementary support to enhance their participation in Medication-Assisted Treatment and community-based services.
  • Offer health coaching and recovery assistance, particularly in areas such as self-advocacy, wellness management, medication compliance, smoking cessation, entitlements, and daily living skills.
  • Facilitate transportation for clients to medical and other appointments, ensuring their engagement and follow-through.
  • Deliver essential services to clients, including benefits enrollment and other program-specific needs.
  • Maintain organized and timely documentation, records, statistics, and reports in accordance with established policies and procedures.
  • Coordinate care planning with other service providers to ensure collaborative, goal-oriented care, including transitions of care.
  • Serve as a resource for team members regarding substance abuse issues.
  • Engage in both telephonic and in-person outreach, service planning, and engagement within the community.
  • Connect clients with community resources, including medical, behavioral, residential, and entitlement services as outlined in their care plans.
  • Oversee service delivery to clients, ensuring coordination and continuity while advocating with service providers as necessary.
  • Provide crisis intervention and follow-up support.
  • Complete Quality Improvement (QI) training during onboarding and participate in ongoing QI training as required.
  • Act as a liaison between the organization and the communities served.
  • Represent the organization professionally and knowledgeably.
  • Assist in promoting the organization through educational outreach about its services to the community.
  • Engage directly with courts, attorneys, hospitals, probation, parole, and various community groups to facilitate referrals to the organization.
  • Report to and collaborate with the director(s) on outreach efforts.
  • Represent the organization at community events, including training sessions and forums.
  • Document and report community contacts made, including names, agencies, and contact information.
  • Operate a company vehicle as necessary.
  • Identify and implement quality improvement initiatives as part of professional development goals.
  • Foster a culture of inclusion that values diverse perspectives and experiences among employees, enhancing decision-making and innovation.
  • Adhere to safety protocols to maintain a secure working environment for all staff.
  • Perform other duties as assigned.

QUALIFICATIONS:

  • High School Diploma or GED.
  • Certified Recovery Peer Advocate (CRPA) Certification required (or provisional certification upon hire with full certification within one year).
  • Required knowledge of medication-assisted treatment, opiate overdose prevention, and substance use disorder recovery services.
  • Relevant personal experience, such as living with a disability or a history of substance use, is essential for serving as a role model for clients.
  • Preferred knowledge of community resources and outreach strategies.
  • Valid New York State Driver's License with a clean DMV record.
