POSITION SUMMARY:

The Community Engagement Coordinator plays a vital role within an individual's support team, focusing on outreach, assistance, and care coordination for eligible clients. This position serves as a mentor and health advocate, promoting the organization and its initiatives within the community. The Coordinator will collaborate with internal staff and external organizations to raise awareness about substance use prevention services.


KEY RESPONSIBILITIES:

  • Conduct outreach to clients to enhance their involvement in services and provide additional support for successful participation in Medication-Assisted Treatment and community-based services.
  • Offer health coaching and recovery assistance, particularly in areas such as self-advocacy, wellness management, medication compliance, smoking cessation, benefits access, and daily living skills.
  • Facilitate transportation for clients to medical and other appointments to ensure their engagement and follow-through.
  • Deliver concrete services to clients, including assistance with benefits and enrollments as required by program needs.
  • Maintain organized and timely documentation, records, statistics, and reports in accordance with policies and procedures.
  • Coordinate care planning with other service providers to ensure collaborative and goal-oriented care, including transitions of care.
  • Serve as a resource for team members regarding substance abuse issues.
  • Provide both telephonic and in-person outreach, engagement, and service planning in the community.
  • Connect clients with community resources, including medical, behavioral, residential, and entitlement services as outlined in their care plans.
  • Oversee service delivery to clients to ensure continuity and coordination; advocate with service providers as necessary.
  • Deliver crisis intervention and follow-up support.
  • Complete Quality Improvement (QI) training during onboarding and participate in ongoing QI training as required.
  • Act as a liaison between the organization and the communities served.
  • Represent the organization professionally and knowledgeably.
  • Assist in promoting the organization through education and information dissemination about its services to the community.
  • Engage directly with courts, legal representatives, hospitals, probation, parole, and various community groups for referrals back to the organization.
  • Report to and collaborate with the director(s) on community engagement efforts.
  • Represent the organization at community events, including training sessions and forums.
  • Document and report community contacts made, including names, agencies, and contact information.
  • Utilize a company vehicle as necessary.
  • Identify and implement quality improvement initiatives as part of professional development.
  • Foster a culture of inclusion that values diverse perspectives and experiences among employees.
  • Adhere to safety protocols to maintain a secure working environment for all staff.
  • Perform other related duties as assigned.

QUALIFICATIONS:

  • High School Diploma or GED.
  • Certified Recovery Peer Advocate (CRPA) Certification (or provisional certification upon hire with full certification required within one year).
  • Understanding of medication-assisted treatment, overdose prevention, and substance use disorder recovery services is essential.
  • Relevant personal experience, such as living with a disability or overcoming substance use challenges, is required to effectively support clients.
  • Familiarity with community resources and outreach strategies is preferred.
  • Valid New York State Driver's License.
  • Clean driving record.
