JOB DESCRIPTION

Insight Global is seeking dedicated Case Coordinators to join a client committed to delivering extensive support and services to veterans in need. Their mission is to empower veterans to lead meaningful lives by providing assistance with housing, healthcare, employment, and other vital services. The Case Coordinator will offer tailored support to veterans, guiding them through various services and resources to enhance their quality of life. This position demands strong interpersonal skills, a profound understanding of veterans' challenges, and the capability to collaborate with multiple agencies and service providers.

Key Responsibilities:


• Evaluate clients' needs, strengths, and obstacles, and collaborate with clients to formulate effective housing stability strategies.


• Deliver comprehensive case management, including regular home visits, housing counseling, client-centered advocacy, referrals, and crisis intervention to foster housing stability.


• Manage an assigned caseload of veterans and/or veteran families, encompassing traditional, Shallow Subsidy, and HUD-VASH cases.


• Clearly communicate the Priority Veteran program to potential clients, landlords, and the community, while conducting outreach in designated areas.


• Maintain consistent communication with local homeless coalitions, VA hospitals, shelters, and other relevant organizations.


• Respond promptly to all inquiries and accurately perform eligibility assessments in accordance with grant guidelines.


• Keep precise and timely case documentation.


• Adhere strictly to grant regulations and program policies.


• Engage actively in professional development training.


• Participate in weekly staff meetings and individual supervision sessions.

Working Conditions:


• Services are delivered in the office, within the community, and in clients' homes.


• Occasional after-hours or weekend work may be required.


• Travel is necessary.

Essential Qualifications:


• Experience in case management, with knowledge of trauma-informed care and person-centered practices.


• Exceptional interpersonal and communication abilities.


• Proficient computer skills, including Microsoft Office suite and Client Management Systems.


• Valid Driver's License and reliable transportation (willingness to travel locally approximately 70% of the time).


• Ability to build rapport with clients experiencing homelessness or at risk of homelessness.


• Successful completion of background checks, including criminal record, drug screening, and TB testing.

Preferred Qualifications:


• Bachelor's Degree in Social Work or a related field is preferred.


• Familiarity with Housing First principles, the Veteran's Administration (VA), and/or U.S. Military experience is advantageous.
