Position Overview

The Ann B. Barshinger Cancer Institute at Lancaster General Health is in search of a skilled Oncology Nurse Coordinator to enhance our team of committed Nurse Navigators.

The successful candidate will have a solid understanding of gastrointestinal cancers, exceptional care coordination abilities, effective communication skills, and a collaborative mindset. A dedication to the growth of the cancer institute and program development, along with resilience and the capacity to manage multiple responsibilities, is essential.

Job Details

LOCATION: Ann B. Barshinger Cancer Institute

HOURS: Full Time (40 hours per week), Monday to Friday (8am-5pm).

Flexibility is required for monthly early morning and/or evening commitments for team meetings and community events.

ROLE SUMMARY: Responsible for delivering clinical navigation for designated patients within the Lancaster General Health system. This role encompasses both clinical and administrative tasks to facilitate the coordination of inpatient and outpatient care needs. Patient care will include adults (19-64 years) and geriatric patients (65 years and older).

KEY RESPONSIBILITIES:
  • Exhibit proficiency in applying nursing principles, evidence-based practices, and therapeutic techniques to perform nursing duties safely and effectively.
  • Possess knowledge of the pathophysiology of specific diseases and comprehend treatment options including surgery, therapy, symptom management, and advanced care planning.
  • Implement current clinical guidelines in the development of treatment plans.
  • Support the physical, mental, and spiritual well-being of patients while collaborating on their treatment plans.
  • Ensure continuity of care throughout the patient journey, serving as a consistent point of contact.
  • Respond appropriately to unsafe practices to protect patient health.
  • Maintain a healing environment, adhering to infection control protocols.
  • Facilitate referrals to local, regional, and national supportive resources.
  • Provide comprehensive treatment summaries and care plans post-treatment.
  • Educate patients and caregivers on disease management, medication, diet, and rehabilitation.
  • Assess patient health needs using a holistic approach and develop personalized care plans.
  • Engage in professional nursing organizations and stay informed on patient care trends.
  • Document nursing activities accurately and collaborate effectively with healthcare team members.
  • Guide patients through the healthcare system, addressing potential barriers to care.
  • Build relationships with physicians and staff across specialties.
  • Screen patients for eligibility in clinical trials to advance practice.
  • Advocate for patients' rights and ensure equitable access to services.
  • Identify opportunities for quality improvement within the service line.
  • Participate in community outreach and educational programs, including health awareness initiatives.
ADDITIONAL RESPONSIBILITIES:
  • Contribute to committees and multidisciplinary boards as appropriate.
  • Perform other duties as assigned.

QUALIFICATIONS

Minimum Required Qualifications:
  • Current Registered Nurse licensure from the PA State Board of Nursing.
  • CPR Certification from the American Heart Association (AHA) required within 30 days of hire.
  • Good standing as a healthcare provider with Medicare, Medicaid, and other health insurance programs.
  • Bachelor's degree in nursing or a related field.
  • Certification in a relevant specialty required within three years of hire.
  • Three or more years of clinical nursing experience in a related specialty.
Preferred Qualifications:
  • Five or more years of clinical nursing experience in a related specialty.
  • Master's degree in Nursing (MSN).
  • Experience in care coordination and patient education.
