Key Responsibilities
  • Provide community-based outreach and engagement-focused case management to ensure linkage and relationship with ongoing providers is secure.
  • Work with care teams to deliver current programming and fill vacant position duties.
  • Collaborate with larger outpatient teams of providers to support clients in actively working towards treatment goals.
  • Coordinate care, such as scheduling, attending, and/or arranging transportation for appointments and other case management needs.

The estimated salary range for this role is $33.00-$40.11 per hour based on experience and qualifications.
