Key Responsibilities

The OCRP LEAD Case Manager will be responsible for:

  • Providing oversight and supervision to assigned staff, ensuring they have the necessary training, resources, and support to deliver high-quality services.
  • Coordinating care with internal and external departments, including scheduling appointments, arranging transportation, and documenting client information.
  • Developing and implementing individualized plans, ensuring that clients receive the most effective and efficient services possible.
  • Ensuring medications are supplied, administered, monitored, and documented appropriately for all clients.
