About the Role

We are seeking a dedicated Patient Support Coordinator to join our team at the Downtown Emergency Service Center.

The successful candidate will have a passion for providing exceptional support to individuals experiencing homelessness, and a commitment to helping them achieve their goals.

Job Description

The Clinical Support Specialist works closely with our integrated care team to provide and coordinate clinical services for tenants. This includes case management, mental health services, and coordination with other providers as needed.

  • Foster strong relationships with clients, engaging them in creative and culturally sensitive ways.
  • Develop and maintain individualized service plans, outlining goals and strategies for housing success.
  • Coordinate case management elements, including hands-on assistance with daily living tasks and teaching independent living skills.
  • Maintain regular contact with clients and case managers to monitor progress and ensure adequate provision of services.

**Key Responsibilities:

  • Provide individual counseling and information referral services to clients as needed.
  • Facilitate community meetings and organize activities to improve client skills in activities of daily living.
  • Participate in staff meetings and trainings.
  • Perform other duties as assigned.

We offer a competitive salary of $62,000 per year, plus benefits including:

**Requirements:

  • Current Agency Affiliated Counselor (AAC) license in Washington State, or equivalent.
  • Clinical experience working with homeless adults disabled by chronic alcoholism, mental illness, and drug use.
  • Knowledge of local homeless, mental health, and chemical dependency service systems.
  • Ability to communicate and work effectively with diverse populations.
  • Commitment to the philosophy of cooperation and continuity across agency programs.

Educational Requirements: A relevant Bachelor's degree in social work, psychology, or related behavioral science, or 1 year of relevant paid work experience with demonstrated ability to perform required job duties.
