Advanced Family Services Coordinator

The role of the Advanced Family Services Coordinator involves the direct provision, coordination, and oversight of professional social work services aimed at supporting children, youth, families, and adult special populations. The primary focus is to address and resolve issues related to client abuse, neglect, dependency, antisocial behavior, exploitation, and family dysfunction, particularly among economically disadvantaged groups.
Key Requirements
To qualify for this position, candidates must meet specific selective requirements, including:
- A valid driver's license.
The essential functions of this role encompass a range of critical activities, including but not limited to:
- Advocating for the health, safety, and rights of clients and their families, while assisting them in accessing necessary services.
- Conducting interviews with clients and their families, as well as relevant professionals, to collect comprehensive personal, social, and background information. This process involves collaborating closely with clients and families to accurately identify their challenges, recognize underlying factors, and leverage available resources and strengths to facilitate problem resolution.
- Evaluating eligibility for various social service programs provided by the agency or accessible within the community.
- Working in partnership with clients and families to develop and implement tailored service or treatment plans, which outline specific goals, methodologies, and expected outcomes. This includes aiding clients and families in securing community, social, and therapeutic resources necessary for achieving these objectives.
- Facilitating individual and family counseling sessions to promote effective social development and address specific challenges.
- Monitoring progress towards established goals and making informed recommendations regarding plans such as reclassification, discharge, aftercare, emergency protective placements, guardianship, or options counseling.
- Preparing detailed social histories, progress reports, evaluations, and discharge summaries, as well as maintaining comprehensive records and files related to assigned clients.
- Enforcing court orders and conditions of community supervision as mandated, and participating in court hearings and other judicial proceedings to provide insights, recommendations, and progress updates regarding clients.
- Intervening in crisis situations, which may include potential suicides, assaults, or cases of sexual abuse, and coordinating with law enforcement and emergency personnel as necessary. This may also involve the need for physical intervention in extreme cases.
- Being available for emergency duty, which may require being on call during designated periods.
Applicants must demonstrate competence in the following areas:
1. At least one of the following qualifications:
- Two years of experience in professional health or human services.
- An Associate's degree accompanied by one year of experience in professional health or human services.
- A Bachelor's degree or higher.
2. Six months of experience in health or human services with a focus on family safety and welfare, including identifying issues, barriers, and goals; determining eligibility for community resources; assisting clients and families in accessing services; resolving family conflicts; and assessing progress towards goals.
3. One year of experience in case management, which includes assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet individual human service needs.
Qualifications for Master Family Services Coordinator
Applicants must demonstrate competence in the following areas:
1. At least one of the following qualifications:
- Three years of experience in professional health or human services.
- An Associate's degree accompanied by two years of experience in professional health or human services.
- A Bachelor's degree or higher.
2. One year of experience in health or human services with a focus on family safety and welfare, including identifying issues, barriers, and goals; determining eligibility for community resources; assisting clients and families in accessing services; resolving family conflicts; and assessing progress towards goals.
3. Two years of experience in case management, which includes assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet individual human service needs.
Additional Information
Applicants must be legally authorized to work in the United States. A satisfactory criminal background check is required as a condition of hire. This position is organized under a collective bargaining agreement.
