Key Responsibilities:
  • Schedule and ensure timely completion of patient assessment visits, including all Oasis visits.
  • Assign and schedule clinicians efficiently, optimizing geography and skill matching.
  • Collaborate with field clinicians to ensure all patient visits are completed as directed.
  • Manage incoming calls from patients and field staff regarding scheduling concerns.
  • Refer clinical and performance-related issues to the clinical manager.
  • Monitor hospitalized patients and ensure appropriate follow-up by the team.
  • Support the clinical manager in maintaining effective communication with patients, clinicians, referral sources, and other personnel.
  • Generate relevant reports and respond to workflow, taking necessary actions.
  • Assist with the transfer of patients between care centers and/or hospice services.
  • Maintain the supply closet, ensuring supplies are up to date and appropriately packaged.
  • Perform additional duties as assigned.
Qualifications:
Required:
  • High School diploma or GED equivalent.
Preferred:
  • One year of scheduling experience in a healthcare setting.
  • One year of administrative experience in a healthcare environment.
