Key Responsibilities- Provide direct patient care in accordance with a comprehensive care plan, ensuring the highest level of comfort and well-being.
- Document patient services accurately and in a timely manner, maintaining the integrity of our records.
- Assess and educate patients and their families/caregivers on various aspects of care, promoting independence and self-management.
- Collaborate with our interdisciplinary team to coordinate home care services, ensuring seamless transitions and optimal outcomes.
- Participate in ongoing professional growth and development opportunities, staying up-to-date with industry best practices and regulatory requirements.
- Ensure compliance with all state and federal laws and regulations, upholding the highest standards of quality and safety.
- Graduate of an accredited practical or vocational nursing program, with current licensure to practice in the state of operation.
- At least one year of nursing experience, with three years preferred.
- Experience in community health, hospice, or med/surg settings is highly desirable.
- Commitment to clinical and documentation excellence, with a focus on delivering exceptional patient care.
