About the Role

We are seeking an experienced Palliative Nurse Practitioner to join our team at HopeHealth Hospice & Palliative Care.

This is a full-time position with a competitive salary range of $120,000 - $150,000 per year, depending on experience.

Job Description:

The Palliative Care Nurse Practitioner will work in collaboration with referring providers and consultants to provide relief of suffering for patients and families facing serious illness. They will assess and recommend relief from burdensome symptoms such as pain, dyspnea, anxiety, and fatigue.

  • Direct Patient Care
  • New consults and follow-up consultations for seriously ill adults in various settings, including homes, assisted living facilities, nursing facilities, hospitals, and clinics.
  • Pain assessments and management recommendations for seriously ill adults.
  • Assessing and managing non-pain symptoms, including nausea, fatigue, dyspnea, and anxiety.
  • Assessing patient disease trajectory and comorbidities, and formulating prognosis in conjunction with treating physicians.
  • Working as a seasoned consultant, fostering positive relationships with referring medical providers or entities.
  • Making recommendations regarding further treatment and end-of-life options based on prognosis and patient/family goals.
  • Counseling and coordinating care with patients, families, providers, and facilities to improve clinical outcomes.
Required Skills and Qualifications:
  • Graduated from an accredited master's degree program in Nursing and/or Clinical Doctorate program in nursing.
  • Certified as a Family/Geriatric or Adult Nurse Practitioner and certified as a Registered Nurse Practitioner in the state of RI required.
  • Minimum one to three years' experience as a nurse practitioner or physician's assistant, preferably in palliative care.
  • Experience with pain and symptom management highly preferred.
  • Knowledge of third-party reimbursement procedures and requirements helpful.
  • Current RI and MA licenses as RN and Registered Nurse Practitioner or Physician's Assistant.
  • Valid driver's license with automobile insurance in accordance with state and/or agency requirements.
