Palliative Care Nurse Practitioner Job Description

Job Summary

The Palliative Care Nurse Practitioner will be responsible for:

  • Providing expert care to seriously ill adults in various settings, including home-based care, hospitals, and long-term care facilities.
  • Developing and implementing individualized care plans that address the physical, emotional, and spiritual needs of patients and their families.
  • Working closely with multidisciplinary teams to ensure seamless care coordination and communication.
Professional Requirements
  • A minimum of 2 years of experience in palliative care or a related field.
  • Advanced education and training in palliative care, nurse practitioner certification, or equivalent.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
