We are seeking a highly skilled Palliative Care Coordinator to join our team at HopeHealth in Massachusetts or Rhode Island. The estimated salary range is $60,000 - $85,000 per year.

Key Responsibilities:

As a Palliative Care Coordinator, you will be responsible for providing timely, responsive customer-centered service to referral sources, patients, patient families, and others contacting HopeHealth regarding our services.

  • Enter and process patient referrals in a timely, complete, and accurate manner.
  • Serve as a liaison between patients, the caregiving staff, and the billing department, supporting all three with comprehensive chart management and communications.
  • Work closely with Palliative Provider Services to ensure that referrals are complete and accurate and obtain any additional information needed to process the referral.

To succeed in this role, you must be able to perform each essential duty satisfactorily. You will need two years of related experience and a high school diploma plus additional specialized training. We also prefer Associate degree, medical assistant, and/or medical secretary background.
