Hospice and Palliative Care Specialist

As a Hospice and Palliative Care Specialist at HopeHealth, you will play a vital role in promoting our organization's mission to provide compassionate and high-quality care to patients and their families in Rhode Island.
This is a full-time position, working 8:00 am - 4:30 pm, eligible for a sign-on bonus up to $2,500. We offer a competitive salary range of $70,000 - $90,000 per year.
Responsibilities:
- Develop and maintain relationships with healthcare professionals, patients, and families to promote HopeHealth's hospice and palliative care services.
- Present information about our services to patients, families, and referral partners in a clear and concise manner.
- Work closely with the Access Center to provide best-in-class customer service and transition patients to the appropriate care.
Qualifications:
- Registered Nurse required; BSN preferred
- RN license required in RI and MA
- Minimum of 5 plus years of clinical experience
- Excellent nursing assessment, clinical, and communication skills
