Hospice Care Specialist
HopeHealth is a not-for-profit healthcare organization providing palliative and hospice care services in Massachusetts and Rhode Island. As a Hospice Care Specialist, you will be part of a team dedicated to delivering compassionate end-of-life care to patients and their families.

**Job Summary:**
The Hospice Care Specialist will provide skilled nursing care to patients with terminal illnesses, focusing on pain management, symptom control, and quality of life enhancement. This role requires excellent clinical assessment skills, strong communication abilities, and a commitment to patient-centered care.

**Responsibilities:**
* Assess the physical, emotional, and spiritual needs of patients and develop individualized plans of care
* Collaborate with interdisciplinary teams to coordinate patient care and address complex needs
* Provide education and support to patients, families, and caregivers on disease management, self-care techniques, and medication administration
* Document patient progress, update care plans, and communicate with physicians and other healthcare professionals as needed

**Requirements:**
* Graduate of an accredited nursing program (BSN preferred)
* Current licensure as a Registered Nurse in Rhode Island and Massachusetts
* Minimum 1-3 years of clinical experience in hospice or palliative care
* Excellent assessment, communication, and interpersonal skills

**Compensation:**
This full-time position offers a competitive salary range of $95,000 - $120,000 per year, depending on experience, plus benefits including medical, dental, and vision insurance, retirement plan, and paid time off.
