Job Description
The Palliative Care Advanced Practice Provider (APP) plays a critical role in our team, providing primary and urgent care services to nursing center residents in collaboration with other medical providers. This position requires a strong focus on pain and symptom management, psychosocial support, and advance care planning for patients navigating serious illnesses.
Responsibilities:
- Provide comprehensive palliative care services to nursing center residents
- Collaborate with the primary care team to develop and implement individualized plans of care
- Offer guidance and education to patients, families, and healthcare teams regarding risks and benefits of relevant interventions
- Serve as a consultant within the interprofessional team to provide recommendations for patient care
Required Skills and Qualifications
To be successful in this role, you will need:
- A graduate degree from an accredited School of Nursing program or Physician Assistant program
- Certification as a Nurse Practitioner or Physician Assistant
- Licensure as a registered nurse or physician assistant in good standing
- Experience working with adult or geriatric populations, preferably with a minimum of two years
- Palliative care certification is preferred
