Job Overview

At HopeHealth, we're seeking an experienced Registered Nurse to provide exceptional end-of-life care to patients in their homes across Rhode Island.

This is a full-time position with 8am-4:30pm hours and a rotating weekend commitment. The successful candidate will be passionate about delivering high-quality care and working collaboratively with the healthcare team.

About the Role

As an End of Life Care Specialist, you'll conduct initial and ongoing assessments of patients, collaborate with physicians to manage patient care plans, and evaluate plans on a regular basis to meet changing needs.

  • Conducts thorough patient assessments to identify needs and develop effective care plans.
  • Collaborates with physicians to ensure seamless care coordination and optimal patient outcomes.
  • Evaluates care plans regularly to ensure they remain current and responsive to patient needs.
  • Provides ongoing training and education to patients and families to empower them in their care.
What You'll Need

To be successful in this role, you'll require:

  • A strong desire to deliver compassionate and patient-centered care.
  • Graduate of an accredited nursing school; RN required.
  • Licensed as a Registered Nurse in Rhode Island and Massachusetts preferred.
  • Minimum of 1-3 years of clinical experience.
