Palliative Services Nurse

**Job Responsibilities:**
- Assess patients' eligibility for hospice services and complete admissions in compliance with Medicare/Medicaid and state regulations.
- Develop and implement comprehensive plans of care, incorporating Local Coverage Determinations and regulatory requirements.
- Collaborate with multidisciplinary teams to ensure seamless care transitions and effective support for patients and families.
- Provide education and support to patients, families, and caregivers regarding hospice services and end-of-life care options.
Qualifications: To succeed in this role, you should possess:
- A current licensure as a Registered Nurse in Rhode Island and Massachusetts.
- A minimum of 1-3 years of experience in hospice or palliative care, with a focus on patient-centered care.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
