Care Coordinator Specialist

The Care Coordinator Specialist will provide exceptional customer service to patients, families, and referral sources while ensuring timely and accurate processing of palliative referrals and hospice admissions.
This role requires a compassionate and organized individual with excellent communication skills to work in a fast-paced environment.
Salary Range: $45,000 - $60,000 per year (dependent on location and experience)
Job Responsibilities- Provide timely and responsive customer service to patients, families, and referral sources
- Process palliative referrals and hospice admissions accurately and efficiently
- Collaborate with the Revenue Cycle team to ensure timely and accurate payer information is entered into the Electronic Health Record
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination
- Bachelor's degree in Healthcare Administration or related field
- 2+ years of experience in healthcare administration or patient access services
- Excellent communication and organizational skills
- Able to work in a fast-paced environment
