Support Services Facilitator
Part-Time Position

Overview:
The Support Services Facilitator plays a crucial role in delivering direct assistance through the execution of Individualized Support Plans. This position is designed for individuals who are passionate about enhancing the lives of those they support.

Key Responsibilities:
  • Ensure the safety and comfort of individuals receiving support through direct supervision and care.
  • Accompany individuals on community outings, appointments, and recreational activities to promote community engagement.
  • Plan and develop inclusive community activities that foster participation.
  • Build and maintain positive relationships with local businesses to enhance community integration.
  • Assist individuals with daily living tasks and personal care routines.
  • Encourage and promote skills that lead to independent living.
  • Collaborate in the creation and implementation of Individualized Support Plans, providing necessary training and assistance.
  • Support individuals in achieving their personal goals and improving their living skills.
  • Oversee individuals during program hours and community activities to ensure their health and safety.
  • Complete required documentation in accordance with organizational policies and procedures.

Qualifications:
  • Strong interpersonal, written, and verbal communication skills, along with effective time management and organizational abilities.
  • Proficient in computer use, with familiarity in agency operating systems and relevant software.
  • Regular access to the internet is necessary.
  • Ability to demonstrate sound judgment in both routine and emergency situations.
  • Physical capability to perform required tasks, including lifting up to 50 pounds.

Education and Experience:
  • High School Diploma or GED, with a preference for candidates with experience working with individuals with developmental disabilities.
  • A valid driver's license that meets the agency's insurance requirements.
