Job Summary

We are seeking a highly skilled and compassionate Housing Support Specialist to join our team at Compass Career Solutions. As a key member of our staff, you will play a vital role in supporting clients in achieving their goals and improving their overall well-being.

Key Responsibilities
  • Client Assessment and Support: Conduct thorough assessments and interviews with clients to determine their eligibility for our services and develop personalized action plans to address their needs.
  • Case Management: Develop and maintain accurate and up-to-date case notes, action plans, and other relevant documents to ensure seamless communication and coordination with clients and stakeholders.
  • Life Skills Development: Provide ongoing support and guidance to clients in developing essential life skills, such as budgeting, time management, and independent living, to enhance their quality of life and independence.
  • Intervention and Referral: Identify and address potential risks and challenges faced by clients, providing targeted interventions and connecting them with relevant community services, including medical, mental health, and counseling services.
  • Benefits Navigation: Assist clients in navigating the process of applying for and qualifying for benefits, such as SSI, SSDI, Medicaid, Medicare, and SNAP, to ensure they receive the support they need.
  • Advocacy and Community Engagement: Act as a strong advocate for clients, connecting them with community resources and services, and representing Compass Career Solutions at professional meetings and events.
  • Reporting and Data Management: Track and report outcomes, providing regular data updates as directed, to ensure the effectiveness of our services and inform program development.
Requirements
  • Strong Communication and Interpersonal Skills: Excellent verbal and written communication skills, with the ability to build strong relationships with clients, colleagues, and community partners.
  • Case Management Experience: Proven experience in case management, with a focus on supporting clients with complex needs and developing personalized plans to address their challenges.
  • Knowledge of Community Resources: Familiarity with local community resources, including medical, mental health, and counseling services, as well as benefits and entitlement programs.
  • Technology Proficiency: Ability to use assigned technology, including laptops, cell phones, and designated software, to effectively manage client data and communicate with stakeholders.
