Role Responsibilities:

The Housing Support Specialist collaborates with clients experiencing mental health challenges as part of a comprehensive treatment team. This role is crucial in facilitating and coordinating services both within and outside our organization. Responsibilities include educating clients about mental health, assisting them in achieving their personal goals, and fostering skill development to encourage independence and recovery.

This position focuses on housing support and requires extensive collaboration with community partners, including housing authorities and local organizations. The specialist will oversee the referral process for various housing options.

The Housing Support Specialist will engage in two primary areas of work: 1) providing home and community-based support to eligible individuals, which encompasses coaching, advocacy, and ongoing assistance; and 2) identifying housing opportunities through outreach to private landlords and community resources.

Key Duties:

  • Collaborate with clients to create personalized treatment plans addressing various life domains, including Mental Health, Substance Use, Daily Living Activities, Employment/Education, Family Support, Financial Stability, Social Connections, Legal Matters, Medical Needs, and Housing.
  • Work on-site at designated housing facilities as needed.
  • Support clients in maintaining their housing and assist with retention strategies.
  • Ensure compliance with all documentation and reporting requirements as per agency standards.
  • Facilitate tenant-landlord mediation and prepare landlords for clients with higher needs.
  • Provide ongoing tenancy support to ensure stable housing for clients.
  • Conduct assessments to identify client preferences for housing.
  • Develop individualized plans for community integration based on assessments.
  • Assist clients in setting measurable short- and long-term goals.
  • Participate in service planning and person-centered meetings.
  • Connect clients with community support services, including healthcare and behavioral health resources.
  • Assist clients with entitlement applications and necessary documentation.
  • Support clients in developing independent living skills.
  • Review and update housing support plans with clients regularly.
  • Facilitate connections to resources that promote successful tenancy.
  • Engage with private landlords and explore grant-funded opportunities for housing placements.

Additional Responsibilities:

  • Support the agency's mission and objectives.
  • Attend scheduled staff and planning meetings.
  • Maintain effective communication and collaborative relationships within the agency and the broader community while upholding confidentiality.
  • Perform other duties as assigned.

Qualifications:

  • Bachelor's degree in Mental Health, Social Work, Psychology, or a related field.
  • 1-2 years of experience in social services, particularly with mental health or substance use disorders, is preferred.
  • 1-2 years of experience in housing support is also preferred.
