Client Support Specialist – Parent-Child Assistance Program

Hourly Rate:
$23.80

Role Summary:


We are seeking a full-time Client Support Specialist for our Parent-Child Assistance Program (PCAP) within the Behavioral Health Department in Vancouver. This role involves working both independently and collaboratively as a vital member of Sea Mar's Behavioral Health team.

The Client Support Specialist will provide direct outreach, home visitation case management, and advocacy services to high-risk mothers who have experienced significant substance use during pregnancy, as well as to their children, partners, and extended families as necessary.


Key Responsibilities:
  • Conduct case management activities in accordance with PCAP policies and procedures, ensuring confidentiality and professionalism.
  • Manage a caseload of 15-20 clients and their families.
  • Perform home visits as clinically required (approximately twice per month).
  • Assist clients in establishing goals based on their individual assessments and identify actionable steps to achieve these goals; monitor their progress.
  • Provide outreach services to connect clients with community agencies that can assist in meeting their needs and objectives.
  • Utilize community and natural supports to enhance services, aiming to integrate clients into their communities and facilitate their graduation from services when appropriate.
  • Administer clinical assessment tools while handling sensitive and confidential information, adhering to PCAP policies.
  • Collect and input data online, coordinate scheduling, and attend case staffing and appointments with community providers involved with the client.
  • Offer housing support and case management services, ensuring proper documentation for housing subcontracts and agreements.
  • Engage in weekly individual supervision with the site Clinical Supervisor to discuss client needs and progress.
  • Participate in weekly team meetings to evaluate client and program outcomes.
  • Conduct outreach to locate and engage clients who may be missing from the program.
  • Attend required training sessions.
  • Obtain and maintain current CPR, Infant CPR, and First Aid Certification.
  • Contribute to report writing, training sessions, and presentations as needed.
  • Ensure the safety, security, and confidentiality of clients and their families.
  • Collaborate effectively with colleagues and personnel from other agencies.
  • Work efficiently in the field with minimal supervision, demonstrating strong time management and problem-solving skills in crisis situations.

Qualifications:
  • Bachelor's Degree is required.
  • Experience in addressing issues related to prenatal substance use or a comparable combination of education and work experience.
  • Proven ability to engage effectively with high-risk mothers and their families.
  • Capability to conduct interviews on sensitive and confidential topics.
  • Strong organizational and time management skills.
  • Excellent written and verbal communication abilities.
  • Proficiency in computer applications (Word, Excel).
  • Access to a vehicle, a valid driver's license, and current auto registration are necessary.
  • Must obtain Basic Life Support (BLS) CPR certification within 90 days of hire and maintain it throughout employment.
