Client Support Specialist – Parent-Child Assistance Program

Sea Mar Community Health Centers is a dedicated community organization focused on delivering exceptional health, human, housing, educational, and cultural services. We are currently looking for a full-time Client Support Specialist for our Parent-Child Assistance Program (PCAP) in Vancouver.

Key Responsibilities:

  • Deliver direct outreach, home visits, and advocacy services to mothers facing significant challenges.
  • Oversee a caseload and perform home visits.
  • Support clients in establishing and reaching their personal goals.
  • Connect clients with community resources and services.
  • Leverage community supports to help clients integrate into their neighborhoods.
  • Conduct clinical assessments and gather relevant data.

Required Qualifications:
