Job Overview

As a Patient Care Coordinator, your responsibilities will include collaborating with patients to assess their unique needs, identifying potential obstacles to care, and crafting tailored care strategies. You will also aid patients in maneuvering through the healthcare system by organizing appointments, coordinating specialist referrals, and addressing insurance and billing inquiries.

The ideal candidate will demonstrate a strong commitment to patient advocacy, possess outstanding communication abilities, and have a genuine desire to positively impact the lives of others. You should be detail-oriented, a proactive problem solver, and capable of managing multiple responsibilities in a dynamic environment.

Key Responsibilities:

  • Establishing rapport with patients and their families to better comprehend their healthcare requirements and offer empathetic support.
  • Guiding patients through the healthcare system by organizing appointments, coordinating specialist referrals, and managing insurance and billing matters.
  • Collaborating with other healthcare professionals and community resources to ensure patients receive comprehensive and integrated care.
  • Providing education and support to empower patients in managing their health conditions and enhancing their overall wellness.
  • Conducting assessments to identify social determinants of health and incorporating these factors into care plans.
  • Documenting patient interactions, care plans, and progress in electronic medical records.
  • Engaging in quality improvement initiatives to enhance care delivery and patient outcomes.

Qualifications:

  • Current active licensure as an LPN, EMT, or Health Educator in Case Management
    OR
    A Bachelor's Degree in Psychology, Social Work, Exercise Science, Health Administration, or a related field
    OR
    2+ years of experience as a medical assistant
    OR
    Currently pursuing a degree in pre-med, pre-nursing, or a related field.
  • Experience in patient-centered care coordination, case management, or care navigation, or a strong desire to assist individuals.
  • Exceptional communication and interpersonal skills, with the ability to build trust and rapport with patients and their families.
  • Strong organizational and time-management skills, capable of effectively managing multiple tasks and priorities.
  • Familiarity with electronic medical records and healthcare technology tools.
  • Understanding of healthcare policies and regulations, including Medicare and Medicaid.
