Position Overview

We are seeking a dedicated Certified Nursing Assistant Care Coordinator to join our innovative team and contribute to our mission of providing exceptional, patient-centered care.

As a Care Coordinator, you will engage with patients to assess their unique healthcare needs, identify obstacles to care, and formulate tailored care strategies. Your role will also involve assisting patients in navigating the healthcare landscape by organizing appointments, coordinating specialist referrals, and addressing insurance and billing inquiries.

The ideal candidate will demonstrate a strong commitment to patient advocacy, possess outstanding communication abilities, and have a genuine desire to enhance the lives of others. You should be detail-oriented, a proactive problem solver, and capable of juggling multiple responsibilities in a dynamic environment.

Key Responsibilities:

  • Establishing rapport with patients and their families to gain insight into their healthcare requirements and providing empathetic support.
  • Delivering self-management education to empower patients in managing chronic conditions effectively at home.
  • Collaborating with patients and primary care providers to develop personalized care plans that incorporate SMART goals.
  • Managing an assigned patient caseload to ensure monthly performance objectives are achieved.
  • Documenting patient interactions, monitoring progress in electronic medical records, and utilizing various software systems.
  • Connecting patients with relevant resources both within and outside of EPIC Health System LLC.
  • Proactively identifying, educating, and addressing gaps in care during patient interactions.
  • Reviewing patients' current medications and conducting medication reconciliation to enhance compliance.
  • Ensuring patient confidentiality and adherence to HIPAA regulations.
  • Participating in ongoing process improvement initiatives to enhance patient care and operational efficiency.
  • Providing training to other staff members as necessary.

Qualifications:

  • Valid Certified Nurse Assistant (CNA) certification is required.
  • Exceptional communication and interpersonal skills, with the ability to build trust and rapport with patients and their families.
  • Strong organizational and time-management capabilities, with the ability to effectively prioritize and manage multiple tasks.
  • Familiarity with electronic medical records and healthcare technology tools.
  • Understanding of healthcare policies and regulations, including Medicare and Medicaid.
