Job Overview

The Intake/Scheduling Coordinator plays a crucial role in managing the patient intake process, fostering positive relationships with clients and referral sources, and overseeing insurance verification and authorization procedures.

Key Responsibilities

  • Oversee daily operations related to patient referrals and intake.
  • Ensure adherence to all relevant state, federal, and regulatory guidelines pertaining to home health and hospice services.
  • Assist in enhancing operational methods and procedures to facilitate patient admissions in line with established policies.
  • Build and maintain strong relationships with existing and potential referral sources.
  • Facilitate smooth transitions for patients to home care by preparing for services, initiating care plans, and coordinating with various service providers and agency teams.
  • Maximize third-party reimbursements through active participation in insurance verification and authorization.
  • Contribute to strategic planning by identifying opportunities for enhanced services to meet client needs.
  • Maintain a thorough understanding of agency contracts to ensure compliance during patient admissions.
  • Stay informed about community resources and assist referral sources in accessing these resources when agency services are not available.
  • Support the scheduling and maintenance of patient visit calendars under the guidance of the Executive Director and Clinical Service Coordinator.
  • Assist in onboarding new staff as needed.
  • Provide liaison services as directed by the Executive Director.
  • Participate in the on-call schedule as assigned by the Clinical Service Coordinator.

Qualifications

  • Minimum of three to five years of experience in healthcare, preferably in home care settings.
  • Strong knowledge of home health and hospice regulations, eligibility criteria, and private payer practices.
  • Excellent communication, negotiation, and public relations skills.
  • Ability to work autonomously, with strong organizational skills, flexibility, and a cooperative attitude.

Job Type: Part-time and Full-time positions available
