Job Description

**Job Summary**

Sand Sage of West Plains is seeking a highly skilled and compassionate Resident Care Coordinator to join our team. As a key member of our Assisted Living Department, you will play a vital role in ensuring the highest level of care and services for our residents.

Key Responsibilities:

  • **Coordinate Care Services**: Ensure the provision of services to residents and monitor compliance with federal and state policies governing resident care.
  • **Staff Supervision**: Supervise and coordinate departmental schedules, ensuring adequate staffing in accordance with company standards, policies, and state law.
  • **Staff Development**: Oversee continuing education and staff development programs for caregivers and medication aides, ensuring compliance with federal and state licensing requirements.
  • **Resident Care**: Conduct or participate in orientation for new staff members, ensuring they are knowledgeable of departmental goals, policies, and procedures.
  • **Medication Management**: Coordinate prescription orders with doctors' offices and pharmacy delivery of medications, ensuring timely and accurate dispensing.
  • **Safety and Security**: Maintain a safe and secure environment for all staff, residents, and guests, following established safety standards.
  • **Communication**: Act as a care manager and resource person for residents and their families, communicating with physicians, family members, and other stakeholders as needed.
  • **Data Entry**: Complete all required data entry into the Eldermark system, ensuring maintenance of resident care information.
  • **Budgeting**: Assist with budget preparation for the department and monitor department costs on a day-to-day basis.
  • **Clerical Duties**: Perform day-to-day clerical work connected with the position, including maintaining records and reports.

Requirements:

  • High school diploma or equivalent.
  • Certifications as required by the state.
  • Prefer one (1) year experience providing residential care to the elderly or similar experience with specialized dementia care programs.
  • Prefer six months supervisory experience in a health-related field.
  • Must be Advanced Certified Medication Aide.
  • Must be Certified Nurse Aide.
