The ideal candidate will evaluate, strategize, organize, and monitor care for individuals with intricate health and social challenges, ensuring they access essential services efficiently and effectively.

Key Responsibilities:

  • Performing in-person evaluations and follow-up communications
  • Managing care for individuals with ongoing health issues
  • Assisting in transitions between different care environments
  • Informing members about their healthcare needs and available resources
  • Recording details of case management activities

Required Qualifications:

  • Bachelor's or Master's degree in Social Work or a related discipline
  • Experience in long-term care or similar healthcare environments
  • Registered Nurse or Licensed Practical Nurse with pertinent experience
  • High school diploma with substantial experience in case management

Preferred Qualifications:

  • Background in home-based clinical or case management
  • Certifications such as CCM or social work licenses
  • Experience working with diverse populations and healthcare settings

Essential Skills:

  • Adaptability in work hours
  • Competence in PC-based software applications
  • Dependable transportation for travel purposes
  • Familiarity with electronic documentation systems

Compensation Range:

$57,000 - $106,700.00
