Job Summary
We are seeking a Full-time Care Coordinator to join our team in Oklahoma City, OK. As a Care Coordinator, you will play a critical role in ensuring Ennoble Care provides high-quality care services. You will work closely with clinicians, staff, and patients to reach healthcare goals and maintain open communication.
Responsibilities
- Care Planning and Coordination: Develop and implement individualized patient care plans, utilizing Ennoble Care's electronic medical record system.
- Patient Support and Education: Provide regular contact with patients to offer care coordination, support, and guidance on managing their care plans.
- Documentation and Communication: Accurately document all client communications and maintain open communication with Primary Care Clinicians and designees regarding patient needs and care plans.
- Team Collaboration: Work closely with cross-functional teams to meet the needs of Ennoble Care's patients, their families, and partner facilities.
- Technology and Documentation: Develop skills in using technology, including secure email, telephone systems, and electronic medical records.
- Adherence to Protocols: Adhere to documentation protocols and best practices for daily work logs, escalation of client issues, and internal communications.
- Customer Service: Demonstrate excellent customer service skills, as evidenced by positive feedback from patients and families.
- Team Support: Contribute to a positive team environment by supporting colleagues in a productive and constructive manner.
Equipment Operation
- Utilize a computer, telephone, copy machine, and other office equipment as necessary.
Qualifications
- Comfortable with phone-based communication for extended periods.
- Compassionate and empathetic towards patients, with exceptional customer service skills.
- Ability to take accurate notes and document tasks in a timely manner.
- Ability to multitask and prioritize tasks while maintaining organization and efficiency.
- Ability to thrive in a fast-paced environment.
- Proficient in using a computer, including Outlook and other Microsoft Office programs.
- Familiarity with basic healthcare terms, conditions, roles, and care principles is beneficial.
- Candidate must pass a drug screen, background check, and possess a positive attitude, adaptability, and a willingness to learn new skills.
