About the Role

Key Responsibilities

  • Provide Psychosocial Support: Offer emotional and resource support to members, caregivers, and care managers to access local resources and services.
  • Monitor Progress and Collaborate: Monitor progress towards care plan goals and collaborate with healthcare providers to revise plans as needed.
  • Document and Maintain Records: Collect, document, and maintain all member information and care management activities to ensure compliance with regulatory requirements.
Requirements
  • Education and Experience: Bachelor's degree and 2-4 years of related experience, or equivalent experience acquired through accomplishments of applicable knowledge, duties, scope, and skill reflective of the level of this position.
  • Licensure and Certification: Current state clinical license preferred.
