Key Responsibilities- Develop and Assess Care Plans: Evaluate member needs, barriers to care, and resources available to recommend and facilitate the best possible outcomes.
- Coordinate Care Services: Identify providers, specialists, and community resources needed for care and coordinate between members, families, and care providers to ensure timely access to services.
- Provide Psychosocial Support: Offer emotional and resource support to members, caregivers, and care managers to access local resources and services.
- Monitor Progress and Collaborate: Monitor progress towards care plan goals and collaborate with healthcare providers to revise plans as needed.
- Document and Maintain Records: Collect, document, and maintain all member information and care management activities to ensure compliance with regulatory requirements.
- Education and Experience: Bachelor's degree and 2-4 years of related experience, or equivalent experience acquired through accomplishments of applicable knowledge, duties, scope, and skill reflective of the level of this position.
- Licensure and Certification: Current state clinical license preferred.
- Competitive Pay: $22.07 - $37.25 per hour.
