GENERAL DESCRIPTION

Under the direction of the Scattered Site II Senior Manager, the Scattered Site II Case Manager is responsible for delivery and coordination of services to residents and ensures their needs are addressed in a timely and appropriate manner.



SPECIFIC RESPONSIBILITIES

1. Provides on-site support to individuals and families residing in the SSII housing program.

2. Assists with new resident orientation prior to program admission.

3. Works with residents, Scattered Site II Senior Manager, Intake coordinator and Senior Mental Health Counselor to develop individual service plans (ISPs). Conducts annual reviews and update of ISPs.

4. Maintains a checklist regarding supportive services delivery and on-site services with Scattered Site II Senior Manager.

5. Addresses resident issues and concerns and/or emergencies in accordance with established policies. Seeks support from Scattered Site II Senior Manager as needed.

6. Identifies resources if resident's health deteriorates and additional services are needed.

7. Responsible for maintaining resident files in accordance with program and funding requirements to ensure overall agency compliance and to assure resident confidentiality is maintained.

8. Is on-call for client/complex emergencies, twenty-four hours a day, seven days a week.

9. Assists and provides direction to residents when troubleshooting issues between resident, neighbors, and property management.

10. Prepares and submits reports to Scattered Site II Senior Manager and other Jerusalem House staff in specified time. Maintains program statistics and records and compiles demographic

11. Provide hands-on case management within a resident's home. i.e., apartment inspections, well-care checks, etc.

12. Identifies additional resources within the community and brings gaps in services to the attention of the Scattered Site II Senior Manager.

13. Assists as needed with the preparation of proposals and grants related to Scattered Site II program.

14. Attends and actively participates in Jerusalem House staff, Program Advisory Committee, community, and other meetings (in-service training/workshops) as required.

15. Performs other duties as assigned.

16. Some evenings and weekends will be required.



MINIMUM QUALIFICATIONS

• College degree preferred and/or minimum five years social service experience

• Valid Georgia driver's license

• Vehicle in good working condition

• Ability to pass criminal background check

Knowledge of:

• HIV/AIDS and related issues

• Problems, issues and appropriate practices related to housing individuals and families

• Community services available to people living with HIV/AIDS

• Procedures and community resources available for emergency assistance

Demonstrated ability to:

• Knowledge of Microsoft Word, Excel and Power point.

• Professionally represent the organization.

• Anticipate, recognize and effectively resolve problems and/or conflict

• Supervise projects, manage and coordinate people.

• React quickly and calmly in emergency situations

• Comply with ongoing reporting and monitoring requirements as indicated



Demonstrated ability to communicate effectively and tactfully:

• With all levels of staff, volunteers, community and collaborative partners

• With the public

• In preparing written and oral reports
