DeKalb Community Service Board (CSB) is a pioneering organization dedicated to providing community-oriented behavioral health and developmental disabilities services in the Atlanta metropolitan area. We serve over 11,000 individuals annually, focusing on those who are uninsured or underinsured, and offer a comprehensive array of mental health services, developmental disabilities programs, and substance abuse treatment.
Position Overview
The Community Case Management Specialist plays a crucial role within the Targeted Case Management (TCM) Program. This position is designed to deliver recovery-oriented services, including case management and psychosocial rehabilitation, to adults facing chronic homelessness or those recently released from institutional settings. Under general supervision, the Specialist will implement interventions aligned with the treatment objectives specified in the Individual Recovery Plan (IRP), facilitate care coordination, and engage in community meetings to enhance service delivery and family support.
Key Responsibilities
• Conduct comprehensive community needs assessments for assigned clients.
• Actively participate in interdisciplinary treatment team meetings, providing insights based on client interactions.
• Deliver ongoing support and case management services tailored to the strengths, needs, and preferences identified in the treatment plan.
• Ensure timely and high-quality documentation of all service interactions, adhering to agency and regulatory standards.
• Regularly meet with the program manager to discuss professional development and case management service evaluations.
• Maintain consistent outreach to all clients on the caseload, ensuring contact at least once per week.
• Facilitate communication among various community service providers on behalf of clients.
• Effectively communicate with agency staff regarding client challenges and concerns.
• Uphold ethical standards and promptly report any safety or health concerns to the program manager.
• Responsible for transporting clients during community integration activities using agency vehicles.
• Additional responsibilities as assigned by the Program Manager or Director.
Work Environment
• Community-based setting, Monday through Friday, with potential for evening or weekend hours as needed.
• Service delivery may occur in-person or via telehealth/telephone.
• Proficiency in using technology, including computers, telephones, and electronic medical records, is essential.
Qualifications
• Bachelor’s degree in social services, human services, social work, or a related field.
• Strong technology skills for utilizing electronic medical records and telehealth systems.
• Experience in care coordination to assist clients in accessing community resources.
Preferred Experience
• At least two years of experience in residential services.
• A minimum of twelve months of experience in a behavioral health setting or a comparable combination of education and experience.
Interpersonal Skills
• Ability to engage professionally and therapeutically with clients and their families.
• Effective communication skills with colleagues and external stakeholders.
