Position Overview
The Case Management Specialist plays a crucial role within the Targeted Case Management (TCM) Program, delivering recovery-oriented services that encompass case management and psychosocial rehabilitation for adults facing chronic homelessness or those recently discharged from healthcare facilities. Under general supervision, the Case Management Specialist will implement interventions aligned with the treatment objectives specified in the Individual Recovery Plan (IRP), facilitate care coordination, engage in community meetings, and participate in system evaluations to enhance family navigation and support services. Additionally, the role includes providing education to families and promoting the use of natural community supports.

Key Responsibilities

  • Conduct comprehensive community needs assessments for all clients on the caseload.
  • Actively participate in interdisciplinary treatment team meetings, offering insights into client treatment needs based on observations and interactions.
  • Deliver ongoing support and case management services in accordance with the identified strengths, needs, and preferences within the treatment plan to help clients achieve their goals.
  • Ensure timely and high-quality documentation of all service interactions, including face-to-face meetings and outreach efforts, adhering to agency and regulatory standards.
  • Meet regularly with the program manager to enhance professional development and evaluate case management service delivery.
  • Maintain weekly outreach to all clients assigned to the caseload.
  • Facilitate communication among various community service providers on behalf of clients.
  • Effectively communicate with clinicians and agency staff regarding client challenges and concerns.
  • Uphold ethical standards and promptly report any safety or health concerns to the program manager.
  • Transport program participants for community integration activities using agency vehicles.
  • Perform additional duties as assigned by the Program Manager or Director.
  • Contribute to interdisciplinary treatment teams by providing input based on client observations.
  • Gather information from consumer families, reports, and other staff to assist in implementing treatment plans.

Working Conditions
  • Community-based setting, Monday through Friday, 8:15 AM to 5:00 PM.
  • Service delivery through in-person and telehealth/telephone methods.
  • Flexibility to work evenings or weekends may be required for community events.
  • Proficient use of computers, telephones, fax machines, and scanning equipment is necessary.
  • Completion of agency-sponsored defensive driving course and other required training is mandatory.

Minimum Qualifications
  • Bachelor's degree in social services, human services, social work, or a related field.
  • Proficient technology skills for using electronic medical records, computers, telehealth systems, and mobile devices.
  • Strong care coordination abilities to assist individuals in accessing additional resources and services.

Preferred Experience
  • Two years of experience in residential services.
  • At least one year of experience in a behavioral health setting or a comparable combination of education and experience.

Interpersonal Skills
  • Ability to engage professionally and therapeutically with clients and their families.
  • Effective communication skills with co-workers, internal and external stakeholders, and the public.
