Assumes a variety of clinical and distributive functions. The pharmacist will be responsible as a proactive health care team member, striving to identify, resolve and/or prevent potential and/or actual drug related problems.

Minimum Education

  • Graduate of an accredited school of pharmacy with a Bachelor of Science degree or Doctor of Pharmacy (Pharm.D.)

Minimum Work Experience

  • Previous experience as a registered pharmacist in a hospital preferred.
  • Completion of PGY1 Pharmacy residency preferred.

Required Licenses/Certifications

  • Vermont pharmacy license or eligibility for licensure by examination or reciprocity.

Required Skills, Knowledge, and Abilities

  • Excel in Microsoft office suites such as Word and Excel.
  • Recognizes and reports practice deviations to improve the medication system.
  • Communication with pharmacy, nursing, medical staff and other health professionals (within the hospital and community) will be significant.
  • Must be able to demonstrate the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety and related criteria, appropriate to the age of the patients served. The skills and knowledge needed to provide such care may be gained through education, training or experience.
