As a key member of our healthcare team, the successful candidate will be responsible for providing high-quality patient care, medication management, and clinical support services.

Key Responsibilities:

  • Provide clinical and distributive pharmacy services to patients, including medication therapy management, patient education, and medication reconciliation.
  • Collaborate with healthcare teams to identify and resolve medication-related problems, and to improve patient outcomes.
  • Develop and implement medication use policies and procedures to ensure safe and effective medication use.
  • Participate in quality improvement initiatives to enhance patient care and safety.
  • Provide education and training to healthcare staff on medication-related topics.

Requirements:

  • Graduate of an accredited school of pharmacy with a Bachelor of Science degree or Doctor of Pharmacy (Ph.D.)
  • Current Vermont pharmacy license or eligibility for licensure by examination or reciprocity.
  • Previous experience as a registered pharmacist in a hospital setting preferred.
  • Completion of PGY1 Pharmacy residency preferred.
