About the Role

As an LVN or LPN at Belmont Village, you will administer medications, support wellness initiatives, help ensure regulatory compliance and assist with quality monitoring. You will communicate with healthcare practitioners, ancillary providers, pharmacies, family members, and staff as needed ensuring timely clinical interventions and follow-up. You will oversee Medication Technicians and Caregivers while providing direction on daily shift assignments.

Typical Responsibilities

  • Follow up on physicians orders, coordinate services with ancillary providers and report to physicians and families any changes of condition or other pertinent clinical care information
  • Provide medication administration to an assigned group of residents according to physicians orders and report any adverse reactions or clinical observations documenting as necessary
  • Ensure resident care and service plans are followed by overseeing tasks of Medication Technicians and care providers
  • Make appropriate situational decisions and manage conflict in a positive and constructive manner
  • Update and review resident medical records, clinical documentation, and incident reports

Requirements

  • Active LVN or LPN license in the State in which this community is located
  • CPR Certified or ability and willingness to obtain within 30 days of hire
  • Proficient in verbal and written English with strong oral and written communication skills
  • Ability to work the defined schedule for this position which may include weekends and holidays
  • Intermediate computer and mobile device skills

  • Strong leadership and supervisory abilities, adept at guiding and directing teams
  • Works with a sense of urgency while paying close attention to detail
  • More than one year of medication management experience
  • Direct supervisory experience
