Position Overview

As a vital member of our healthcare team, the LPN Assessment Nurse is responsible for conducting comprehensive evaluations and follow-ups for clients enrolled in the elderly and disabled waiver program.

Core Responsibilities

  • Perform initial and annual evaluations in the client's residence, assessing their capacity to perform Instrumental Activities of Daily Living (IADLs) and Activities of Daily Living (ADLs).
  • Conduct modified assessments as necessary for clients transitioning from hospitals or nursing facilities, as well as for those requiring additional services.
Ensure that clients receive necessary services to maintain their independence within the community, adhering to established guidelines to meet their needs.

  • Maintain regular communication with the program administrator to stay updated on re-assessment timelines for SOURCE and CCSP annual reviews.
  • Review client medications for suitability, documenting any changes in prescriptions, dosages, or discontinuations.
  • Accurately record assessment findings in the Harmony system, following agency and regulatory standards.
  • Request medical and hospital documentation as needed to support eligibility determinations.
  • Engage in weekly team meetings with the Medical Director to discuss new admissions, reassessments, hospitalizations, and any concerns regarding client health or functional status.
  • Participate in mandatory training sessions to enhance skills and knowledge.
  • Adhere to corporate compliance protocols and report any job-related hazards, including exposure to bodily fluids.
  • Follow established safety procedures, including fire safety and infection control measures.
Required Knowledge, Skills, and Abilities

  • Proficient in computer usage with the ability to learn new software applications.
  • Ability to meet deadlines set by the Department of Community Health (DCH).
  • Effective communication skills, including the ability to work with translators for non-English-speaking clients.
  • Maintain open communication with supervisors regarding daily tasks and challenges.
  • Ability to foster positive relationships with colleagues and management.
  • Commitment to the organization's mission and values, ensuring compliance with all policies and regulations.
  • Confidentiality in handling client and company information.
  • Support the implementation of waiver program guidelines and collaborate with the Medical Director and Home First team.
  • Conduct home visits to provide care and services in accordance with program regulations.
  • Flexibility to adapt to changing needs and manage additional projects as required.
  • Assess the health status of assigned clients in line with agency policies and procedures.
  • Meet productivity expectations while utilizing time and resources efficiently.
  • Fulfill other duties as assigned by the Administrator.
  • Engage in ongoing education through the agency's in-service training program.
Minimum Qualifications

Education: Nursing degree from an accredited institution or completion of an approved Licensed Practical Nurse (LPN) program.

Experience: A minimum of two (2) years of practical nursing experience, particularly with elderly and disabled populations.

Licensure: Current and unrestricted LPN license in the state of practice.

Preferred Qualifications

Three (3) years of practical nursing experience is preferred, along with experience in governmental waiver programs or a combination of relevant experience and training.
